Conversation style Data Capture form

I need a conversation style Data capture form for my website. Preferably using a form builder for wordpress or another CMS to make future changes easy. I have made a demo using Jotform which you can check here - https://form.jotform.com/213140535997056 but jotform can't handle all the functionality i need. So need a bit more customization. For example using jotform i could not populate dropdown options from other text fields on the form. There will be conditional logic required to enable/disable certain fields based on other fields. I need functionality to send the data captured to Zapier if possible or to a web API in future so the choice of plugin has to be made accordingly. Attaching the excel file with all the fields and logic required. The entries will be stored in MySQl database and need either a word or PDF download.
