Connect HTML form with a PHP plugin

I am looking for a freelancer to help me connect an exisiting HTML form with an existing PHP plugin on my WordPress website. The purpose of the HTML form is to collect and store user information.
The form has a section of radio buttons. The options are dynamically fetched using the PHP plugin that already exists. The information collected is sent using the PHP plugin that already exists.

Skills and Experience:
