- **Sex (Radio Button)**: Choose from Male/Female/Other.
- **Designation (Dropdown Menu)**: Provide a list of designations (e.g., Member, President, Secretary).
- **Location Address (Text Field)**: Home or office address.
- **Phone Number (Text Field)**: With validation for correct phone number format.

### 9. Testing
- **Test the Form**: Test the form with a dummy submission to ensure:
- All fields are working.
- Validation is correctly implemented.
- The top 50 eligibile auto-invitations are working as expected.
- The opt-in WhatsApp functionality is sending the brochure correctly.
