Codeigniter Expert Quick & Fast

I have a list of task that i need addition fast. Most of the work is done i just need it to be saved correctly

1. form is done up
2. Need what submit -> you save into database (database is also done)
3. Create a History page of submission with ability to edit

Task 2:

We need this doen:

- Form is already done, need. checkerbox (when ticked some fields will be disabled/enabled)
- When saved need a new field in database to classify between checked and unchecked submission
- Need a list of all submissions and can edit the submission.
