Client Portal
Budget: $250 – $500 USD
Please see attached docs for info on the project I have. We only need the portal in CodeIgniter or something. Budget for this is USD$800. Work over it and let me know if you interested or not. Timeline is 9 December to go live with it.
There are 2 items:
1. Abri-Bezuidenhout-SOW-Final15Nov2022.doc: This one has the details of the section in it and should answer all your questions.
2. Patient Registration Form.png – This is just a sample of the registration form.
Just to summarise the patient portal menu once logged in will be:
Patient information form*** - Will be a form they can complete/update their details on (that is the one attached as png)
Medical history form*** - Will be a form where they can add their medical history: Can add a title, date and description.
Billing policy*** - Just a page where they can view TEXT on billing policy. We can just add a toggle switch at the top of “I agree”
T&Cs & POPIA consent *** - Also just a TEXT page to view the legal requirements etc. We can just add a toggle switch at the top of “I agree”
Endoscopy request* - An online form to be completed by patient, don’t have form yet
Load documents* - Option to load various documents with name, description and date captured.
Request a sick note* - An online form the patient can complete which is mailed to the Drs room to request a sick note.
Load authorisation* - Upload of a document for a medical procedure. Patient will load date, title and document.
Postoperative wound care* - From admin side admin will load a document and assign to a patient which they can see here.
Feedback form* - Online form to mail to Drs rooms
Request an appointment * - Online form to request an appointment, not linked to any calendar etc – just static form.
For the admin side it will be the normal views of
1. View Patients and here you can then view each patients profile and info loaded under the profiles. They can also edit/delete profiles.
2. There will also be 2 text pages for Billing Policy and T&C/POPI that they can edit in the admin section.
3. Must have option to add multiple admin users.
4. Reporting – just a basic option to export users with their contact info to XLS.
