Change existing quiz in php.

Description of what is needed to be changed in the system:

1 - On the home screen, change the fields:

- Name:
- Mobile:
- Email:

for:

- Age (2-digit numeric field)
- Gender (radio button - 2 options: Male / Female)
- Education (select field - 5 options:
a) up to 4ª grade elementary school
b) 5th to 8ª grade elementary school
c) high school
d) higher education
e) complete higher education
)
- Income (select field - 5 options:
a) 1 to 2 salaries
b) from 2 to 5 salaries
c) from 5 to 10 salaries
e) more than 10 salaries
)
- Region / Neighborhood (select field with option to type the neighborhood or choose from the list (there are 90 neighborhoods).

Item 2 - I will send the survey link via whatsapp or email, so I would like the script to have a control or setting to prevent a person from responding to each survey more than once, damaging the result.
This validation can be by the whatsapp number and email that will be used to send the survey links.

Item 3 - it is possible to add more alternatives when creating the surveys, or create questions with deviations, when adding the questions, define whether it will be a normal question or with deviations (conditional) eg:

question 1 I added an option button and set it to be a branched question, with these options:

1 - Which of these fruits do you like best?

a) strawberry
b) pineapple
c) grape

if the person answered 'a', skip to question 2.
if the person answered 'b', skip to question 3.
if the person answered 'c', skip to question 4.

2 - Why don't you like pineapple or grapes?

4 - Why don't you like strawberries or pineapples?
THE:

5 - How would you like to eat these fruits?
