Canine Control Incident Reporting System

As someone implementing a solution for reporting dog control incidents, I am in need of developing a web application that facilitates submission of these reports.

The project is for handling day to day management of animal birth control program of municipal corporations. The process in short is.
• A dog is picked up from the streets and gps details/photo taken.
• Brought to the animal shelter, put into kennels and details recorded with photo.
• Taken for surgery on the next day.
• Surgery details entered by the veterinary doctor.
• Given food for next 3 days – photos taken while giving food
• Given post operative medicine by animal caretaker – anti-biotic and painkiller.
• Despatched by the caretaker into the dog van
• Released back to the area by the catcher.
