CUSTOM MODULE: Free toppings before checkout (woocommerce)

I looking for a custom module for wordpress / woocommerce.
our need: choose free / extras toppings in a "before checkout" step (see mockup)
Free toppings are related/calculated with products & quantities in cart
Need 3 fields in "woocommerce product edition" to define free toppings quantities (as you can see in screenshot)
Toppings/ extras will be displayed in cart items (and final order mail).
