Build a website

A website that acts as a healthcare consultant to the client. I would like the website to ask the client questions in a survey manner. So ask the clients age and gender. Also a search bar where they can type their symptoms in (the search bar has to have a drop down auto fill menu). And I would also like there to be admin access & client login. The admin can edit information that’s on the website. This is a rough idea for now. Thanks.
