Build me a simple portal for survey.

Hi freelancers, I want a survey portal which will consist of two panels namely admin and agent. The admin will have the part to insert questions while agent will just see the questions and options uploaded. Now admin will insert question based on two fields ie age_band and residential after which admin will insert the parent question and then after options. Again admin will insert the child questions and options with respect to the parent question. I am uploading a working concept of the project of agents view below. You can check it and let me know if you can do this. I will need the source code and sql file
