Build me a one page web application

Ignore the navbar and hero title. It's everything starting from "Vehicle Type" and under all the way to the bottom of the form. I have attached all the screenshots to make it clear.
The purpose of the page I need use to design to a selection choice form and after the user fills out the fields with their contact information after they click send the information will go to my email.
I need the frontend to be HTML,CSS and bootstrap.
Backend - Use MYSQL for the database and PHP.
Once the user selects all the products and fills out the form I need the form to be sent to the email address of my choice which I will specify when you get closer to finishing the single web page application. Therefore, my email mail will show the results of the form after the user clicks submit. For the images you can use place holders to make it easier for you, I can edit the code and add the images. In regards to the text please make sure it is the exact same as the website link I have linked. Ensure the webpage is fully responsive please.
For the prices and hours of each product, please put the price as £100 for each product and one hour for each product if a user selects any product. I'm also a developer but I don't have time to complete this as I'm working on other projects so I'm the middle man within out project. Furthermore, in terms of the deadline the quicker the better but don't feel the need to rush I have five days max for you to deliver this to me. I will submit three days hoping you can do it within three days. If this is successful we can work more as I have more project work I want to outsource to a reliable developer.
