Build me a calculator on Joomla website

For a client of me I'm looking for a programmer who can help build a simple calculator on a Joomla website. Preferebly in RSForms if possible. The calculator is part of a quote request for a loan.
First field is a free field where the visitor can say how much they want to spend every month for the loan (e.g. 50 euro). Second field is a dropdown where the visitor can select how long they want the loan (24, 36, 48 or 60 months).
When the visitor filled in both fields the outcome should be monthly amount x the selected time + a percentage of the total outcome (24 months is 12%, 36 months is 13%, 48 months is 15% and 60 months is 17%). The next page should be to fill in some personal information and send the quotation by mail.
