Build me a billing app
Budget: $250 – $750 CAD
I would like a web-app that can be hosted on a LAMP stack which can be used by a medical professional (physiotherapist, nurse, massage therapist, etc) to track the patients they see, the clinics they work at, and the services they provide. The app should also be able to pull various reports based on user-selected parameters.
Users must be able to login, and metadata about transactions (user ID, datetime) must be captured for each record at creation and at update.
Data Capture
1. Capture Appointments
We need to capture appointment date, patient name/ID, clinic, and service type.
2. Capture Clinic information
We need to capture clinic name, address, billing contact person, and billing contact email. Billing frequency would be nice too, for future invoice Automation.
3. Capture Service Types
We need to capture service name, timing (30 mins, 60 mins, etc), effective date, end date, clinic name, and earnings (can be % or fixed $ or a mix of both). Should also support a negative amount (for example, commission could be 50% of service rate minus $1.50/appointment)
Reports
1. Invoice
A list of all the fees payable to the physiotherapist for a given timeframe.
2. Patients per Clinic - per day, and average over timeframe
3. Visits/Service Type/Patient
4. Visits/Patient
5. Total hours worked per Clinic - per day, and sum over timeframe
