Build me an appointment calender

I want patient name to be in the daily appointment calendar "Daily" or the monthly calendar "Monthly" as I put in the patient name, date of appointment, and time of appointment in "List." In other words, I do not want to put patient information two or three times. Please let me know if you need further detail.
