Build a Single Form in PHP AJAX

Dear All,
Need to design a single form in PHP Ajax with the form validation. please find below the process:
1. Client will enter his ID or registration ID
2. If the ID does not match an error will be displayed “Invalid Information ”
3. If ID matched, the Name will be automatically displayed in English.
4. Client will click on the checkbox to confirm the account ownership
5. Link to the pdf document will be available to view the details of the acquisition
6. Checkbox will be available to confirm the disclaimer
7. Finally the client has to click on yes or No to activate the submit button
8. Once submit is clicked, the client will receive a OTP on the registered Email or via mobile.
10. Upon entering the correct OTP the form will be submitted
11. Date and time of submission will be recorded along with the owner confirmation check, disclaimer check and participation answer “Yes / No”
Need to complete task in one to two days maximum.
