Build a "Request for Time Off" Program

I am looking to create a local web-hosted program with a database backend that employees can walk up to, type in their employee number and 4 digit pin, and submit a request for time off. At the same time, I also need a "Supervisor" panel that an employee's supervisor can go to and approve or deny the time off. There would also need to be reporting as well on both the employee and supervisor's side. On the employee's side, they can see their history of submissions and see what ones are approved. On the supervisor's side, they can run a specific employee's history or a time frame report as well.
