Build a Form Based System with Location GIS Built in

Hi,
I am looking for a form based system where we can custom build forms for specific tasks for companies.
Ideally this need to be created with a mapping tool. So when user inputs information they can also add location data to the map with a polygon, point or line functionality in 2D format.
The form will require a database initially small but then can be built out big. If succesful there will be lot more work to come out of this.
