Birthday SMS & Email Reminder - HTML Form > mySQL DB > Twilio SMS & PHP email sender

Hi there, I need a basic HTML form with 2 fields, 1 which captures a users next birthday date e.g 01/01/2023 AND mobile phone number/email address. This data then gets submitted via PHP to a mySQL DB. A cron job, will execute a PHP script every day, that will send an API call to Twilio to send a SMS message OR if a user provided an email address it will send out an email reminder. This will occur 5 days prior to the users birthday.
The completed project will include the following:
1) SQL commands to create the DB
2) PHP script
3) Basic Instructions on how to setup script and test.
