Auto-populate form fields from API

I need the ability to auto populate form fields that are on a specific site. This information will come from my database therefore you would have to create some sort of an API to pull over the necessary information. You will also need to create a google extension that will pull the information from the API to auto populate the form fields. The other option would be to auto populate with a specific URL that I will provide.
