Budget: $750 – $1,500 USD
I'm seeking a seasoned developer, adept in PHP, Ajax, jQuery, and MySQL, to create a questionnaire form builder. Key features should include:
1. Storage: The capacity to store all form responses in a database for further analysis.
2. Question Variety: The form should be capable of handling a large number of questions, with a mix of single text fields, paragraph fields, multiple choice (dropdown, checkbox, radio), image upload, video upload, terms & condition agreement checkboxes and digital signature input.
3. The ability to customize each questions settings-- whether it's a required field, whether the question is public or internal only.
4. A responsive interface where questions in the questionnaire can be re-odered by dragging them.
Ideal freelancers would have a proven track record in developing similar platforms and possess solid skills in PHP, Ajax, jQuery, and MySQL. Their bid should also reflect their understanding of database management and UX/UI design. Prior experience in handling file uploads (like images and videos) would also be beneficial.
