Admin Panel using PHP Laravel -- 4

As soon as we click on the service the below page given in image will open, where there will be an order form for each service, and we should be able to make this form with the admin panel for each service. So, if we click on any service this order form will come up which will be made directly from the admin panel for each service. All the data which is being filled in the form should be able to receive in the admin panel.
