Add country & country phone code picker extension

Hi, i have a small project which is to add country code & country picker extension in sign up page. its a SaaS based platform (uscreen.tv). There is option to edit the code in template.
Scope of the project as follows:

1. Add extension of "Flag + code" in Phone Number filed
2. Add extension of "Flag + country" in Nationality filed
Note: After adding we need to check the back end where data is storing or not
