Acuity Scheduling intake form contract

I'm look for someone who can help me get my contract on the Acuity Scheduling intake form. I need the contract/waiver to be listed with a subhead, description of the terms, and then a required check box labeled that it has been agreed to and read for each section.

I have seen this done on another website but I can't do it myself.
