Access Database for Contact & Employee Management -- 2

I am looking for someone who can build an Access database for contact and employee management. I need a simple database (all required fields will be provided).
1-The database must have the capability to collect those fields from the user in a neat-looking form.
2- It should include a search option in the form, which can search the database using either the first or last name and display all the data of that employee on a page similar to the provided template.
