Unique Form Template Design Needed

My name is Daniel. I'm a single multi talented business owner. I'm a grateful Freelancer.com client, just like you, except that I also have a passion: web development.
You see, I've slowly constructed a personal project named TV everywhere (that is the meaning of TéléPartout in french). I've created a non profit organisation that is https://telepartout.org.
My purpose today, as I need help, is to find a designer that can create a form template in preparation of adding a splash screen. The splash screen of the home page is not yet completed, it is being produced as I am writing to you by a beautiful soul. I have the template which I'm sharing with you to give you an idea of what I'm working with.
Make no mistage, I don't have a multi-thousand dollar budget, it is just you and I who will be working on this project.
The ideal candidate is patient, not expensive with a "surprise agenda: I'm going away for a marriage or taking some vacation time, etc
The candidate should know or have previous experience in building a wireframe where you have worked in the past with sys admins and .css integrator in PHP.
I'm looking for a Photoshop expert. Please do not apply if you don't have a full Photoshop Licence.
NO, you don't need to speak french (although some basic would help) and no Photopea / GIMP / Affinity or Canva for that matter is accepted.
The deliverable is:
One template form with, once it is done, different backgrounds total 7-10 different
Formats delivered are .psd deinterlaced, vectorized off course and .ai. Plus I will need a full color version via .jpg and .bmp including a black and white version
