Need graphic designer

Create retail card, combination header-backer for new product. Have sombrero cutout, poly-bag with product staples to it. Includes product title, description. Backer includes use instructions. Assembled size approximately 3" x 8", with 3" x 1-1/2" folded "header".
As this is a new product, graphics designer must be US based and sign NDA.
