11 page PDF Needs to be Fillable Forms

I have an 11 page PDF form that I need to have an Adobe expert make so the forms are fillable, and the check boxes are able to be checked, and the radio buttons filled in. I also need a logo I will provide to be put in the top left area of the document. Attached is the current PDF.
