I need a PDF expert

We use form that are saved as a PDf in another program. We would like to open this pdf and apply a template with several drop down form fields and also text fields while still seeing the original PDF below. Then be able to print with both the original and template onto one paper. Also be able to save it as a pdf afterwards.
