I need a PDF expert

I am a mental health therapist and recently opened my own private practice. I have between 10-20 pdfs that I need to be made fillable and publishable (I think). I don't know anything about this and I've tried for hours using PDFescape and PdfSimpli. The forms will be added to my patient portal for me to send to patients electronically. I want them to be able to click/type their responses directly on the form and submit it. I don't want them to have to download, reply and upload. This is a very urgent, time-sensitive need. I uploaded an example of the types of forms I need. Thank you in advance for any help!
