I need a PDF to Generate a Lease Contract based on form fields and check boxes

I need a PDF to Generate a Lease Contract based on form fields and check boxes. The source of form fields and lease contract terms and conditions text I want is here: https://bit.ly/3xynaSS - basically I want to copy all form fields and generated leased contract from there. The PDF should have a first part with form fields and a second part where the Lease Contract is generated based on the form field answered in first part.
