make a intake form to get information from client , and then fill a pdf automatically with the data from the form

My project involves creating an intake form to gather information from my clients. This form will then be used to fill in a PDF document that already has a specific format. To ensure an accurate and efficient filling process, I will be leveraging Adobe Acrobat, as it's automation tool of choice. The form will be designed to collect specific personal information from my clients.
The attachment is the document i need in a web intake form. i need to be able to save the client file after created in pdf form.
