fillable form that will auto populate another form. form will bill filled out by multiple people

I am looking to create a fillable form that will automatically populate another form. The form that needs to be filled out would be in the form of a PDF. I have a detailed layout for the form in mind already, and it would need to be filled out by between 30-40 people each month. The completed forms would then need to be used to populate another form.
