fillable form creation

I am looking for a freelancer to create a fillable insurance application form in PDF format. I already have a design in mind that I will provide. The ideal candidate should have experience in creating fillable forms and be proficient in PDF editing software. The project will require attention to detail and accuracy. The deliverable should be a fully functional PDF form that can be easily filled out and submitted.

I have a document that we will start from and a partially edited sample to start the ball rolling.
I will provide a list of additional fields (classes with check boxes) that need to be integrated with the form

once that phase is fleshed out, there will be a second round of edits.
