fillable PDF form

I am in need of a fillable PDF form for customer sign up when they first come to our business so we can create a client profile for them. The document attached called ''Astute Fact Find 1-3'' is what we use currently and are happy with how it works with the fillable boxes, dates, tick boxes and auto calculations for dollar amounts. We want to change the design to make it look more professional and less home made. The other document of ''Fact Find New and Fact Find New PDF'' are the new fields we would like and the layout of the document.

We are really happy with the new layout we created however it still looks a bit homemade, plus we need all the fillable boxes created on the PDF once it has been designed to look more professional (See Astute Fact Find 1-3 for how we want the fillable boxes).
