fill in the blank contract

i want to make a rent/lease agreement that is a fill in the blank form. I have all the terms and conditions. i need someone to design a form in pdf or word format. where someone can easily jump from space to space to fill in the appropraite fields. also i would like the contract to look somewhat aesthetically pleasing.
