Word or PDF Template with ability to scan in barcodes

I want to create a Template/Form where I can input the persons mailbox #, then scan in the packages received using the barcode scanner to put in the tracking numbers. Have the person sign it. It would need to have the current date already on the form. I need to be able to "Save as" then return to the blank template and start again.
YOU will need to have a barcode scanner so you can verify the information will upload into the document. You can test it with any package that has a tracking number.
