Update dates on our PDF document

Hello,
We need three dates on the attached PDF document updated and the font etc should all match the rest of the document—we can't locate the word document to update on our end.
At the top right where it says Date: it should be: June 5, 2024
Just a little further down, it says: Effective: I need that to be 24-05-05
And below that it says, Expiry: I need that to be 25-05-05
