Transform PDF Questionnaires to Google Forms

I am looking to have my PDF questionnaires converted into Google Forms. This will enable me to collect responses electronically and efficiently.
The forms contain simple questions (Yes/No, Multiple Choice) that need to be accurately reflected in the Google Forms. The forms should also mirror my brand’s look and feel; hence, they should include my company's logo and colors.
