Secure Signature PDF Form Development

I need a skilled professional to create a fillable, secure signature PDF form using Adobe. This project involves the development of a form that will collect medical information, hence it is critical that the form is designed with utmost security and confidentiality in mind.
Key Features Required:
- Signature fields: The form must have designated areas for signatures. Make signatures fields secure and un editable once signed.
- Text Input Fields: There should be multiple text input fields for the collection of personal medical information.
- Check Boxes: The form should also incorporate check boxes for easy selection and response.
- More space between the first paragraph and Patient Name field
- Note when trying on our own the signature lines were static so form may need to be recreated.
The completed forms will not be sent via email but instead should be designed for upload to a website.
