Quick PDF to Excel Convertion - OCR and Manual Entry NEEDS TO BE EXACT

I have a bad scan that I can't run through regular OCR methods. I'd like for you to convert the PDF into an Excel table and validate the output. There are only three columns that NEED to be exact: "Document Number", "Document Date" and "Document Amount".
