Problem with pdf form that dont work fine in google chrome

I have a form that I have created in adobe acrobat, it is in pdf, but in google chrome it does not work well, the checkboxes are not marked or they do not appear marked.
If it is created with openoffice (form XML) it works fine, but I don't have time to create one exactly the same.
Also, I have the document in .doc word.
