PDF to word

I'm looking to have a document converted from PDF to Word format. Specifically, 1 page of text need to be converted. Please retain the exact set out of the original document. Please find the color logo from the internet. The document is in PDF format, and I need them to be converted into Word format. Once in word please convert to PDF and add fillable fields where required.
If you have previous experience in this type of task, then I would love to hear from you.
