PDF contract and questionnaire

I need something my clients can complete digitally, including a signature. Currently, I send them a PDF file they have to print, complete and scan to return it to me. Here's the Word doc I use.

We can break it into two forms if that makes more sense. In other words, the contract separate from the homework.
