PDF Sheet Duplication & Field Renaming

I have an existing PDF form created in Adobe Acrobat that needs to be modified. The task involves duplicating the input pages 11 times and ensuring that each form field on these new pages has a unique name. The content and structure of the duplicated pages will be identical to the original, but the field names will need to be different in order to facilitate data duplication.
