I am looking for a freelancer who can create a dynamic form for my project.
Type of Form:
- Survey Form
User Data Collection:
- Yes, I need to collect user data through the form - copy format only to paste into text or word document
Email Notifications:
- No, I do not need to receive emails from the form.
Skills and Experience:
- Proficient in creating dynamic forms using PDF format
-Knowledge of LiveCycle to create expandable options/menus
- Familiarity with survey form design and layout
- Knowledge of data collection and storage methods
- Attention to detail to ensure accurate data capture
Unlimited revision but I expect only 2 revisions. I must be able to edit.
