PDF Editable Form

I would like a PDF editable form created from a fixed template (supplied). The form is a field report form which is to be edited using an electronic device in the field, e.g. table or smart phone using PDF software such as acrobat or bluebeam. the example given is simply a copy of the form. I would like all text boxes and check boxes to be clear and ready to be used. if any freelancers have this particular experience, i would also like to discuss possible options. on the second page is a picture, i would like the ability to enter a photograph or picture into the form, which i can then edit with information.
