We have two PDF documents that need to be merged into a single document. Attached are the Part A and Part B documents that need to be merged, Part A first then Part B.
Once merged into a single document we need text fields, date fields and checkbox applied to the various questions that need to be completed to complete the form. The current document doesn't allow for editable fields, however we need the document updated so that fields can be edited.
