Interactive PDF creation

We are looking for someone to create an interactive PDF that we can send to our customers after they order from us.
There will be some basic questions such as
- Have you ordered signs from us before (Yes/No) If they click NO it will goto a new question however if they click yes it pops up with an image for them to choose the layout from
We will design a full brief and supply images in any format required.
There would only be approx 5-6 questions
