Interactive PDF Questionnaire

I have an interactive PDF questionnaire created on ADOBE that is not functioning as it should. The questionnaire has 104 items and many items have 5 option buttons to chose from, with the respondent clicking on their choice. The issue is that a small number of the interactive fields appear to be linked by mistake so that when a person selects one choice on one item, this selection is accidentally duplicated on another item in the questionnaire. So I require a person familiar with creating interactive PDF's to edit the existing questionnaire. The PDF will be provided to the person whom I award the project to. thanks
