We need you to help us create a pdf ready to fill with data using BULK PDF
We need the word or odt file and also the pdf ready to use so we can fill it with BULK PDF
You need to add appendix too

The file need to be clean without border on the form, same font everywhere, same layout etc..
We started to make the file so maybe it's just some editing
